This plan has a partial exclusion.

Exclusion:

Must be 18 years of age or older [surgery] (p. 20); The following surgeries are considered cosmetic when used to improve the gender specific appearance of an individual who has undergone or is planning to undergo gender reassignment surgery and are not covered:

  • Liposuction/body contouring
  • Rhinoplasty
  • Facial bone reconstruction
  • Voice modification surgery
  • Hair removal
  • Face lift
  • Blepharoplasty
  • Reversal of genital surgery
  • Sperm preservation in advance of hormone treatment or surgery
  • Cryopreservation in advance of hormone treatment or surgery
  • Surgical treatment of anyone under the age of 18. (p. 51)
