Historical Notes:

Maryland in 2015 promulgated regulations under COMAR 10.09.02.05 extending HealthChoice Managed Care Organizations coverage for gender confirmation surgery.

Prior to the 2015 amendment, COMAR 10.09.02.05(A)(21) explicitly excluded gender confirmation surgery:
“A. Services which are not covered are: … (21) Gender change or sex reassignment procedures”"

Notes:

The coverage extension excludes some hair removal and facial surgery as outlined on page 3 of the guidance document.
